How to Initiate a Service Request

1. Sign a service agreement with UI including the service offering you are requesting.
   1. If one exists, go to step 2
   2. To initiate a service request that is not in your service agreement, you must contact our Supplier Relations Dept (supplier.relations@uinet.com) to modify the service agreement.
2. Select "Service Request Management" on the SMS web site.
   1. Select a service offering from the drop-down menu.
   2. Select "Initiate Service Request."
   3. Fill out the required information and press submit.

How to Check Service Request Status

1. Select "Service Request Management" on the on the SMS web site.
   
2. Select "View Service Request Status."
   
3. Select a Service Request (the statuses for service request are Open, In Progress, Incomplete, Complete)."
   
4. UI will send an e-mail confirmation when your request has been fulfilled.

Service Offerings Prices

The Department of Public Utility Control (DPUC) determines the prices UI is allowed to charge for each service offering.
