- Billing address (if different from service address)
- Service start date
- Social Security Number (SSN)
- If you choose not to provide your SSN, two valid forms of identification (e.g., driver’s license, military ID, or passport)
- Name and SSN of any other individuals authorized to access your account
- Phone number

- Date the old service will stop (i.e., move-out date),
- Date the new service will start (i.e., move-in date)
- Phone number for new service location.

To prevent identity theft, we require identification when you start service. You may provide your Social Security Number (SSN), or, if you prefer not to submit your SSN online, you must instead provide two valid forms of identification (such as a driver’s license, military ID, or passport) via mail. Failure to provide or validate these within 15 days will result in service disconnection.
